Private Duty Nursing

Private Duty Nursing is a type of home care service that involves skilled nursing care provided by a licensed nurse in the patient's home. Private Duty Nurses (PDNs) provide care that is tailored to meet the specific needs of each patient, including those with chronic illnesses, disabilities, or those recovering from surgery or injury.

PDNs provide a range of nursing services, including wound care, medication management, monitoring vital signs, managing ventilators and tracheostomies, and providing other specialized care as needed.

Private Duty Nursing services can be arranged for short-term or long-term care, and the level of care provided can vary from intermittent visits to 24-hour care. The goal of private duty nursing is to help patients maintain their independence and improve their quality of life while remaining in the comfort of their own home.

Medication Management and Reminders

Home Care Medication Management and Reminders is a type of service provided by healthcare professionals or caregivers to ensure that patients take their medications as prescribed by their healthcare providers. It involves a range of activities that help patients manage their medication regimen effectively, such as:

  1. Medication Reminders: All Caregivers can set reminders to ensure that patients take their medication on time, every time. 

  2. Medication Organization: Nurses can organize medications by sorting them by time and day of the week, so patients can easily identify and take the correct medication at the right time.

  3. Medication Refills: Caregivers can help patients order and refill their medications to ensure that they do not run out of their prescribed medication.

  4. Medication Administration: Nurses can administer medication to patients who may have difficulty taking their medication, such as patients who have difficulty swallowing pills or who need injections.

  5. Monitoring for Side Effects: All Caregivers can monitor patients for any adverse side effects of medication and report any concerns to the healthcare provider.

  6. Medication management and reminders can help patients stay on track with their medication regimen and reduce the risk of medication errors, hospitalizations, and other adverse health outcomes.

The nurse works closely with the child's healthcare team and the school to develop an individualized care plan that outlines the child's medical needs and the nursing care required during the school day. This care plan may include administering medication, monitoring vital signs, providing treatments such as tube feedings or tracheostomy care, and managing medical equipment.

The nurse also provides education and training to the child, their family, and the school staff on how to manage the child's medical needs and respond to any emergencies that may arise.

One-On-One School Nursing in Home Care helps children with complex medical needs to attend school safely and comfortably, ensuring that they receive the medical care they need while still being able to participate in their academic and social activities. It also provides peace of mind to parents and caregivers, knowing that their child is receiving quality medical care during the school day.

One-On-One School Nursing

One-On-One School Nursing in Home Care is a specialized type of home care service provided to children with complex medical needs who require medical assistance during the school day. This service involves a licensed nurse who provides one-on-one medical care to the child while they are at school.

Home Care Services

Home care services refer to a range of healthcare services provided in the comfort of a patient's home to assist them with activities of daily living, medical needs, and emotional support. These services are usually provided by licensed healthcare professionals, trained caregivers, or both.

  1. Personal Care Services: Assistance with activities of daily living, such as bathing, dressing, grooming, toileting, and mobility. Trusting Hands Homecare only employs certified Home Health Aides to provide these services.

  2. Skilled Nursing Services: Medical services provided by licensed nurses, such as wound care, medication management, monitoring vital signs, and administering injections.

  3. Companion Care Services: Non-medical services that provide emotional support and companionship to patients, such as socializing, playing games, or engaging in other leisure activities. Trusting Hands Homecare only employs certified Home Health Aides to provide these services.

  4. Home care services can be arranged for short-term or long-term care, depending on the patient's needs. The goal of home care services is to help patients maintain their independence, improve their quality of life, and reduce the need for hospitalization or institutional care.

Tracheostomy Care & Ventilator Care

Tracheostomy Care and Ventilator Care are specialized home care services that are provided to patients who require assistance with breathing due to a medical condition or injury. These services are usually provided by licensed nurses who are trained to provide specialized care.

Tracheostomy Care involves the management of a tracheostomy tube, which is a tube inserted through the neck into the trachea to help the patient breathe. The care provided may include suctioning the airway, cleaning and changing the tracheostomy tube, monitoring for signs of infection or complications, and providing education and training to the patient and family members on tracheostomy care.

Ventilator Care involves the management of a mechanical ventilator, which is a machine that helps the patient breathe by delivering oxygen and removing carbon dioxide. The care provided may include monitoring the ventilator settings, suctioning the airway, monitoring the patient's vital signs, managing the patient's airway, and providing education and training to the patient and family members on ventilator care.

Tracheostomy Care and Ventilator Care Home Care services are provided to patients who require 24-hour care or intermittent care depending on their medical needs. The goal of these services is to help patients manage their respiratory needs effectively, reduce the risk of complications, and improve their quality of life while remaining in the comfort of their own home.

Assistance With Daily Living Activities 

Assistance with Daily Living Activities in Home Care is a type of home care service that provides assistance to individuals who require help with daily living activities due to age, disability, or illness. These services are usually provided by caregivers who are trained to provide assistance with activities of daily living (ADLs).

Some common ADLs that may require assistance include:

  1. Personal hygiene and grooming: Assistance with bathing, dressing, grooming, and oral hygiene.

  2. Mobility: Assistance with transferring, walking, and using mobility aids.

  3. Nutrition and hydration: Assistance with meal preparation, feeding, and ensuring adequate hydration.

  4. Toileting: Assistance with using the toilet, cleaning, and maintaining continence.

  5. Medication management: Assistance with medication reminders, scheduling, and administration.

  6. Housekeeping: Assistance with light housekeeping tasks, such as laundry, cleaning, and organization.

  7. Assistance with Daily Living Activities in Home Care services are designed to provide support to individuals who wish to remain in the comfort of their own home while receiving the care they need. The goal of these services is to improve the individual's quality of life, maintain their independence, and promote their safety
